MP DMAT How to apply, MP DMAT Registration

Application Forms for entrance examination will be available at the selected branches of State Bank of India on payment of Rs. 750/-( Rs. Seven hundred fifty only) through cash. DMAT Application Forms are also available at all participating colleges/ Association Office on payment of Rs. 750/- ( Rs. Seven hundred fifty only) through cash/ demand draft in favour of APDMC payable at Bhopal. Application Forms can also be obtained from the Association Office/Participating Colleges by post by remitting a DD of Rs. 850/- (including postal charges). DMAT Application Form can also be downloaded from websites. In case the application form has been downloaded from website it shall be submitted alongwith Demand Draft of Rs. 1500/- (for MBBS / BDS Courses) and (Rs. 750/- for other Courses ) drawn in favour of APDMC, payable at Bhopal.

DMAT Application Form important instructions:
The candidate should study the information brochure carefully and understand with all requirements and eligibility. Form should be filled by the candidate in his/her own handwriting. Use Blue/ Black Ball Point pen only. Fill the form in English using CAPITAL LETTERS only.

Do not do overwriting, cuttings, erasing in the application form. It is better to fill in a photocopy of the form first and thereafter fill in the original application form.

Candidates can submit application form in person and pay examination fee in cash in APDMC Office.

Write your complete address in the application form for further communication with APDMC. Keep a photocopy of ORIGINAL filled application form.

You should write name and application form number on the back of demand draft.

Submit the filled application form complete in all respect along with necessary fees to the APDMC, Bhopal.

Name of the candidate: Write first name, then middle name (abbreviate if necessary) and then surname in the boxes provided on top. One box should be left blank between first, second and surname.

Date of birth of candidate: write the date in the two boxes provided in the first column. Write the month in next two boxes (if born in Jan write 01) and write the year (if born in year 1991 write 9 and 1)in the next two boxes.

Gender: Tick Male or Female in the box provided.

Courses: Tick the choice of course. Candidates are eligible for any course available in the counselling if qualify for that course whether opted or not.

Choice of Centres: Tick choice of the two centres in order of preference and write Centre Code No.

Name of Parent/Guardian: Write first name, then middle name (abbreviate if necessary) then surname in the boxes provided.

Mailing Address: Write mailing address clearly inside the boxes. Write your PIN code. Write address in capital letters only.

Contact Address: Write your telephone/mobile numbers in the boxes.

Photograph: A WHITE BACKGROUND recent high contrast 3.5 cm. x 4.5 cm. color photograph should be pasted. Photograph should contain front profile with full face, head, shoulder and with both ears visible and taken without wearing any cap or hat or sunglasses. Polaroid photo is NOT acceptable. Photograph must be taken on or after 1st January, 2015 with a playcard indicating the name of the candidate (as in the application form) and the date of taking photograph.

Photograph should be pasted. Do not pin/staple to fix the photograph. It is not required to attest or sign the photograph.

Candidate should sign himself/herself entirely inside the box provided.

Thumb Impression: – Apply your left thumb impression using BLUE/ RED STAMP PAD ink in the space provided. Allow the ink to get completely dry.

The candidate should keep the photocopy of application form. Photocopy of application form from DMAT office can be obtained on payment of Rs.200/-

If a candidate tries to influence the authorities, directly or indirectly will disqualify the candidate.
Mention your Application Form Number in any correspondence. If Roll Number is allotted, the candidate should mention the Roll Number in all correspondence.
